Payments

Full payment for services and supplies is expected at the time of service.  This includes all co-payments, deductibles, non-covered durable supplies and if we don’t file your particular insurance, the full cost of the visit.

We accept Visa, Mastercard and Discover credit cards, personal check or cash.

For those insurances that we do not file, we can supply you with the filled out insurance form for you to submit to them for reimbursement if you request.

Please bring your valid insurance card with you for this visit so that we can verify your status and the amount that you owe for the visit.

If your insurance requires a referral from your primary care physician for your visit to be covered, it is your responsibility to have the proper referral numbers and paper work completed when you come for the visit.
